They were both taken to Advocate Illinois Masonic Medical Center, where the woman later died, according to police. The Cook County medical examiner's office could not immediately confirm the fatality.

The man was in "grave" condition Sunday afternoon, police said. Police said the shooting may have been gang-related.

Perez, of the 2700 block of West 24th Street, was pronounced dead at the scene, authorities said.

A police source said the victim had documented gang ties.

The weekend's first fatal shooting happened in the West Side Humboldt Park neighborhood Friday night.

About 10:50 p.m., 36-year-old Courtney Jackson and another man were in a car in the 1100 block of North Springfield, when someone walked up and fired shots in their direction, police said. The shooter then ran away.

Both men showed up at the hospital after the shooting.

In the most recent nonfatal incident, three men were shot in another Humboldt Park attack on Sunday afternoon.

About 1:45 p.m., they were outside in the 900 block of North Karlov when they heard shots and realized they'd been struck as they ran away, police said.

At least 16 others have been wounded in shootings since 4:30 p.m. Friday.

Also, an 18-year-old man was shot to death by a Chicago Police officer Friday night in the South Shore neighborhood.

About 10:10 p.m., Grand Crossing District officers on patrol near 71st Street and Paxton Avenue heard shots fired and saw a van speeding away from the area, according to a statement from Chicago Police.

A pursuit ensued, and the officers watched someone jump from the vehicle with a gun in his hand, police Deputy Chief Dana Alexander told reporters at the scene.

The officers chased the man — identified as Jeffery Kemp — to 74th Street and Merrill Avenue and ordered him to drop the weapon, but instead he turned around and pointed it at the officers, authorities said. An officer then fired two shots, striking the man.

Kemp, of the 7400 block of South Jeffery, was dead at the scene, according to the medical examiner's office.

Police said a gun was recovered at the scene. No officers were injured.

The Independent Police Review Authority, which investigates all police-involved shootings, has been notified, spokesman Larry Merritt said.
