1. HOW TO ENTER

a. These rules govern the Make Tonight A DiGiorno Pizza Basketball Night Promotion (the “Promotion”) being conducted by WSCR (AM) 670 (the “Station”) beginning on Wednesday, January 25th, 2012 at 5:00 pm Central Time (“CT”) and ending on Wednesday, February 29th, 2012 at 7:00 pm CT (the “Promotion Period”).

b. Registration starts on Wednesday, January 25th, 2012 at 5:00 PM CT. You may register for the Promotion via the following method:

c. At the on-site event, you will need to obtain an official entry form (available while supplies last) and legibly complete the official entry form in its entirety including your full name, complete address (including city, state and ZIP code), daytime phone number (including area code), e-mail address (if any), and date of birth. Hand the completed official entry form to the Station representative on-site or place the entry form in the designated registration box. The Released Parties are not responsible for lost, stolen, mangled, misdirected, illegible, incomplete or late entries.

d. All entries per on-site event must be received by the end of each of the twelve (12) on-site events during the Promotion Period. Late entries will not be accepted. Entries become property of the Station, and shall not be returned. By participating in this sweepstakes, entrants agree to be bound by these Official Rules and the decisions of the Judges, which shall be considered final in all things.

2. ELIGIBILITY RESTRICTIONS

a. The Promotion is open to all persons who are 18 years of age or older and who reside within 75 miles of the United Center in Chicago, Illinois, which includes counties in Illinois, Indiana, Michigan, and Wisconsin. Employees of the Station, its licensee, Nestle Pizza Division, the Chicago Bulls, CBLS Corp., Chicago Professional Sports Limited Partnership, Chicago Bulls Limited Partnership, the United Center, BSV Limited Partnership, each of their respective parent, subsidiary and affiliated entities, Station’s advertising and promotional agencies, their participating sponsors, other radio stations in the Chicago metropolitan area, the members of their immediate families (spouse, parent, sibling or child) and those living in the same household of each (whether related or not), are ineligible to enter or win. This Promotion is subject to all applicable federal, state and local laws and regulations and is void where prohibited.

b. Only one (1) winner per household is permitted in any Station-conducted promotion. Listeners are eligible to win a prize valued at $600 or more in a Station-conducted promotion only once every six (6) months.

c. Each winner must provide valid government-issued photo identification and provide their complete address, date of birth, phone number and will be required to sign an affidavit of eligibility to claim a prize.

d. Entrants are required to provide truthful information and the Station will reject and delete any entry that it discovers to be false or fraudulent. The Station will disqualify any entry from individuals who do not meet the eligibility requirements.

3. PRIZES
a. Twelve (12) prizes will be awarded in this Promotion, one at each event. Each prize consists of the following: one (1) two-hour long on-court basketball activity conducted by a Chicago Bulls celebrity (broadcaster, coach, former player or assistant coach, subject to availability) to take place at the United Center, located at 1901 West Madison Street, Chicago, IL 60612 on Wednesday, March 14th, 2012 from approximately 3:00-5:00pm CT; one (1) practice jersey or t-shirt; two (2) banquet passes for a buffet-style dinner located in the Governors’ Room at approximately 7:00pm, and two (2) 100-level tickets for winner and guest to the Chicago Bulls home game at 8:30pm CT on Wednesday, March 14th, 2012. Each participant will be responsible for bringing their own toiletries, towels, and soft soled/ non marking shoes for the clinic and shall be required to sign full liability releases in the form presented by the Chicago Bulls. The Approximate Retail Value of each prize is $480.00.

b. Each winner will be solely responsible for all taxes and all other fees and expenses not specified herein associated with the receipt and use of the Prize. The Released Parties accept no responsibility for repairing any real or supposed damage to any prize. Certain restrictions may apply. Prize is subject to availability. If the winner cannot attend the prize event on dates specified by the Station, the winner will be disqualified and an alternate winner may be selected in the Station’s sole discretion. All other costs not specifically stated herein, including but not limited to those of, taxes, meals, tips, parking and transportation to/from the United Center, are the responsibility of the winner. The winner and guest must sign a Publicity and Liability Waiver prior to attending the prize event. If a selected event is unable to take place as scheduled, for reasons such as cancellation, preemption, postponement or unavailability, or for any reason beyond the control of the Station, the Station’s sole responsibility to the winner will be to award the remaining available portion of the prize and no substitution or compensation will be provided for the unawarded portion of the prize. The Station reserves the right to remove or to deny entry to the winner and/or guest if either engages in a disruptive manner or with intent to annoy, abuse, threaten or harass any other person at any prize-related event. The tickets will be subject to the terms and conditions as set forth by the issuer of the tickets.

c. Prizes will be awarded following confirmation of eligibility, and will be awarded to the winner at the United Center, or as otherwise specified by the Station when contacting the winner. In the event that a prize or prize certificate is mailed to the winner, it will be with the prior written consent of the winner and therefore, winner assumes the risk of its loss. The Station is not responsible for the safe arrival of a prize or prize certificate.

d. There is no substitution, transfer or cash equivalent for prizes, except that the Station may, at its sole discretion, substitute prizes or cash of comparable value. The prizes are expressly limited to the item(s) listed above and unless otherwise expressly specified, do not include taxes, gratuities, parking or any other expenses. Other restrictions may apply.

e. Nestle is a prize sponsor only of this promotion, and any issues or concerns regarding the promotion should be directed to the Station.

4. WINNER SELECTION AND NOTIFICATION

a. Decisions of the Station management with respect to the Promotion are final.

b. Twelve (12) winners will be selected on-site at the end of each of the twelve (12) promotional events during the Promotion Period. At the end of each on-site event, the Station will select one (1) potential prize winner through a random drawing from all eligible on-site entries received, Drawings will be held at:

c. The winners do not need to be present on the aforementioned dates at the time of the drawing. The Station will notify each potential winner by phone following each on-site prize winner drawing. Prizes will be awarded upon confirmation of eligibility.

d. Odds of an entrant winning a prize depend upon the number of eligible on-site entries received at each event.

e. Winners need not listen to the Station, nor be present to win. Winner must be able to provide ID as requested above, and be able to sign an affidavit of eligibility.

f. Each winner must execute and return any required affidavit of eligibility and/or liability/publicity release at the time that they are notified that they have been selected as a winner, or prize will be forfeited and an alternate winner may be selected at random. If a potential winner cannot be contacted, fails to sign and return the required affidavit of eligibility and/or liability/publicity release within the required time period, or if a prize or prize notification is returned as undeliverable, the potential winner forfeits the prize.

5. CONDITIONS

a. Payments of all federal, state and local taxes are solely the responsibility of the winner. The winner will be required to complete and submit an IRS Form W-9 with the winner’s full Social Security Number or the equivalent for receipt of any prize valued at $600 or more or for any prizes awarded by the Station (or any other CBS Radio station in Station’s market) in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in forfeiture of the prize. Such winnings of $600 or more will be reported to the IRS.

b. By participating in the Promotion, the winner agrees to have the winner’s name, voice and likeness used in any advertising or broadcasting material relating to the Promotion without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.

c. Prior to awarding any prize, the Station, in its sole discretion, will require Promotion winner and his/her guest(s) to sign a liability release, agreeing to release and hold harmless Nestle USA, the Station (which includes its licensee, the licensee’s parent, subsidiary and affiliated entities), the Chicago Bulls, CBLS Corp., Chicago Professional Sports Limited Partnership, Chicago Bulls Limited Partnership, the United Center, BSV Limited Partnership, their respective officers, shareholders, directors, employees, agents and representatives, any promotional or advertising agencies involved in this promotion, and all of their successors and assigns from and against any and all claims or liability arising directly or indirectly from the prize and participation in the Promotion.

d. If for any reason this Promotion cannot be executed as planned, including, but not limited to, as a result of infection by computer virus, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Station that corrupt or affect the security, administration, fairness, integrity or proper conduct of the Promotion, or if the Promotion is compromised or becomes technically corrupted in any way, electronically or otherwise, the Station reserves the right to cancel, terminate, suspend or modify the Promotion. If the Promotion is terminated before the original end date, the Station reserves the right to select the winners by random drawing from among all eligible non-suspect entries received as of the termination time/date or otherwise modify the procedure for selecting winners.

e. The Station, in its sole discretion, reserves the right to disqualify any person tampering with the entry process or the operation of the Station’s website. Failure to comply with the rules of the Promotion may result in a contestant’s disqualification and/or forfeiture of any prize or prizes. If the Station makes a good faith determination that an entrant has cheated or committed fraudulent activity in connection with a Promotion, the Station reserves the right to disqualify that entrant from entering and/or winning future Promotions and to prosecute and seek damages to the fullest extent permitted by law.

f. The Station reserves the right to make changes in the rules of the Promotion, including, without limitation, the substitution of a prize of equivalent value, which will become effective upon announcement. If due to circumstances beyond the control of the Station, any competition or prize-related event or travel is delayed, rescheduled, postponed or cancelled, the Station reserves the right, but not the obligation, to cancel, terminate, suspend, or modify the Promotion and shall not be required to award a substitute prize.

g. Neither Nestle USA nor the Station are responsible for typographical or other errors in the printing, the offering or the administration of the Promotion or in the announcement of a prize.

h. a. These rules govern the ‘Massage Envy-P of The Week’ Promotion (the “Promotion”) being conducted by WSCR(AM) (the “Station”) beginning on Friday, February 3rd , 2012 at 5:00pm Central Time (CT) and ending on Friday, January 4th , 2013 at 5:00pm CT (“Promotion Period”).

b. To participate in the Promotion, listen to the Station on select Friday’s listed below during the Promotion Period during the 5:00 PM hour for the cue/reminder to enter (the “Cue”). Upon hearing the Cue, you may enter the designated keyword at any time via one (1) of the following methods:

Via Text Message: After the Cue is announced, submit a text message with the key word (as prompted by the on-air cue to text) to the short code “67011″. The Station is not responsible for telephone service outages, delays, equipment malfunctions and any other technological difficulties that may prevent an individual from sending a text message. Message and data rates may apply, and the Station is not responsible for any fees incurred by an entrant for this method of entry.

The Approximate Retail Value (“ARV”) of the prize is $50.00. The winner will be solely responsible for all taxes and all other fees and expenses not specified herein associated with the receipt and use of the Prize. The Station accepts no responsibility for repairing any real or supposed damage to any prize. The gift certificate will be subject to the terms and conditions as set forth by the issuer of the gift certificate. Use of this card constitutes acceptance of the following terms and conditions:
• The gift card may be used to purchase massage services only.
• Massage envy is not responsible for lost, stolen or unauthorized use of this promotional gift card.
• The gift card is not redeemable for cash.

a. These rules govern the Toyota Auto Show Football Legends Dinner Giveaway (the “Promotion”) being conducted by WSCR (AM) 670 (the “Station”) beginning on Friday, February 10th, 2012 at 9:00AM Central Time (“CT”) and ending on Sunday, February 19th, 2012 at 4:00PM CT (the “Promotion Period”).

b. To participate in the Promotion, you may enter via the following method:

ON-SITE ENTRY: Go to one (1) of the following 2012 Chicago Auto Show Appearances (“On-Site Event”), taking place at McCormick Place, located at 2301 S. Lake Shore Drive Chicago IL.

c. At the On-Site Event, entrants will need to fill out an official entry form (available while supplies last) and legibly complete the form in its entirety, by including their full name, complete address (including city, state and ZIP code), daytime phone number (including area code), e-mail address (if any), and date of birth. The completed official entry form must be returned to a Station representative on-site, or entrants can place the entry form in the designated registration box. The Station is not responsible for lost, stolen, mangled, misdirected, illegible, incomplete or late entries.

d. All entries per on-site event must be received by the end of each of the twelve (12) On-Site Events during the Promotion Period. Late entries will not be accepted. Entries become property of the Station, and shall not be returned. By participating in this Promotion, entrants agree to be bound by these Official Rules and the decisions of the Station, which shall be considered final.

2. ELIGIBILITY RESTRICTIONS

a. The Promotion is open to all persons who are 18 years of age or older and who reside in the Chicago Arbitron Metro Area, which includes counties in Illinois, Indiana, Michigan, and Wisconsin. Employees of the Station, its licensee, Toyota Motor Corporation, each of their respective parents, subsidiary and affiliated entities, Station’s advertising and promotional agencies, their participating sponsors, other radio stations in the Chicago metropolitan area, the members of their immediate families (spouse, parent, sibling or child) and those living in the same household of each (whether related or not), are ineligible to enter or win. This Promotion is subject to all applicable federal, state and local laws and regulations and is void where prohibited.

b. Only one (1) winner per household is permitted in any Station-conducted promotion.

3. PRIZES

a. One (1) Prize will be awarded in this Promotion. The Prize consists of the following: one (1) dinner for two (2) people (winner and one (1) guest) with Chicago football legends Steve McMichael and Keith Van Horne, to take place at Hub 51, located at 51 W. Hubbard, Chicago IL 60602. Dinner will take place on Thursday, February 23rd at 6PM CT. Dinner includes one (1) entrée per guest, and a maximum of two (2) beverages per guest, not to exceed a total value of $200.00 for both guests combined. All other costs not specifically stated herein, including but not limited to those of, taxes, tips, parking and transportation to/from the Hub 51, are the responsibility of the winner. Approximate Retail Value of the Prize is $200.00.

b. Each winner will be solely responsible for all taxes and all other fees and expenses not specified herein associated with the receipt and use of the Prize. The Station accepts no responsibility for repairing any real or supposed damage to any prize. Certain restrictions may apply. Prize is subject to availability. If the winner cannot attend the prize event on dates specified by the Station, the winner will be disqualified and an alternate winner may be selected in the Station’s sole discretion. All other costs not specifically stated herein, including but not limited to those of, taxes, tips, parking and transportation to/from the Hub 51, are the responsibility of the winner. The winner and guest must sign a Publicity and Liability Waiver prior to attending the prize event. If a selected event is unable to take place as scheduled, for reasons such as cancellation, preemption, postponement or unavailability, or for any reason beyond the control of the Station, the Station’s sole responsibility to the winner will be to award the remaining available portion of the prize and no substitution or compensation will be provided for the unawarded portion of the prize. The Station reserves the right to remove or to deny entry to the winner and/or guest if either engages in a disruptive manner or with intent to annoy, abuse, threaten or harass any other person at the dinner.

c. Prizes will be awarded following confirmation of eligibility, and will be awarded to the winner at Hub 51, or as otherwise specified by the Station when contacting the winner. In the event that a prize or prize certificate is mailed to the winner, it will be with the prior written consent of the winner and therefore, winner assumes the risk of its loss. The Station is not responsible for the safe arrival of a prize or prize certificate.

d. There is no substitution, transfer or cash equivalent for prizes, except that the Station may, at its sole discretion, substitute prizes or cash of comparable value. The prizes are expressly limited to the item listed above and unless otherwise expressly specified, do not include taxes, gratuities, parking or any other expenses. Other restrictions may apply.

e. Toyota Motor Corporation is a prize sponsor only of this Promotion. Any issues or concerns regarding the Promotion should be directed to the Station.

4. WINNER SELECTION AND NOTIFICATION

a. Decisions of the Station management with respect to the Promotion are final.

b. On Monday, February 20th, 2012 10:00AM CT, the Station will select one (1) Prize winner through a random drawing from all entries received at the On-Site Events during the Promotion Period.

c. Winners need not listen to the Station, nor be present at the time of the drawing to win. Entrants must be present to enter at one (1) of the On-Site Events. The Station will notify the potential winner by phone following the winner drawing. Prizes will be awarded upon confirmation of eligibility.

d. Odds of an entrant winning a prize depend upon the number of eligible on-site entries received at the On-Site Events.

e. Winner must be able to provide ID as requested above, and be able to sign an affidavit of eligibility.
Each winner must execute and return any required affidavit of eligibility and/or liability/publicity release within one (1) day of winning or prize will be forfeited or prize will be forfeited and an alternate winner may be selected at random. If a potential winner cannot be contacted, fails to sign and return the required affidavit of eligibility and/or liability/publicity release within the required time period, or if a prize or prize notification is returned as undeliverable, the potential winner forfeits the prize.

5. CONDITIONS

c. Prior to awarding any prize, the Station, in its sole discretion, will require Promotion winner and his/her guest(s) to sign a liability release, agreeing to release and hold harmless Toyota Motor Corporation U.S.A., the Station (which includes its licensee, the licensee’s parent, subsidiary and affiliated entities), their respective officers, shareholders, directors, employees, agents and representatives, any promotional or advertising agencies involved in this promotion, and all of their successors and assigns from and against any and all claims or liability arising directly or indirectly from the prize and participation in the Promotion. Additional releases may be required by Hub 51.

g. Neither Toyota Motor Company U.S.A. nor the Station are responsible for typographical or other errors in the printing, the offering or the administration of the Promotion or in the announcement of a prize.

h. For a copy of these Official Rules, send a self-addressed stamped envelope to be postmarked by Sunday, February 19th, 2012 to “Toyota Auto Show Football Legends Dinner” – Official Rules,” Attn: Promotions, WSCR 670 AM 180 N. Stetson Suite 1250 Chicago, IL 60601. For the names of the prize winner send a self-addressed stamped envelope to be postmarked by Thursday, April 19th, 2012 to the above address marked “Toyota Auto Show Football Legends Dinner – Winner List.” The Official Rules and the Winner List (when completed) shall also be available during regular business hours at the main offices of the Station and may be posted online at CBSCHICAGO.COM
