NIU’s “Teacher Of The Week”
It’s time for Teacher of the Week sponsored by Northern Illinois University!
If you know a teacher grades K through 12 who deserves recognition for a job well done, nominate them today! We’ll randomly select a nominee each entry period to be recognized as our Teacher of the Week and we’ll give them a special shout out during WBBM’s Pre-Game Show!
Plus they’ll get a $250 Visa Gift Card that they can use for school supplies and be in the running for the Grand Prize – the chance to have a Chicago Football Player come to visit their school! It’s Teacher of the Week, sponsored by Northern Illinois University.

1. HOW TO ENTER
a. These rules govern WBBM’s Teacher of the Week promotion (the “Promotion”) being conducted by WBBM(AM) and WCFS-FM (collectively the “Stations”) beginning on Monday, August 26, 2013 at 5:00pm Central Time (“CT) and ending at 12:00pm (Noon) CT on Sunday, December 29, 2013.
b. To participate in the Promotion, you may log onto http://www.cbschicago.com/contests and follow the links and instructions to enter the Promotion. Submit your first name and last name, complete address, city, state, zip code, telephone number, date of birth and a valid email address in the online entry form. Also submit your nomination for the teacher who you think deserves to be named the Teacher of the Week. Include the nominee’s first and last name, the grade they teach (Kindergarten through 12th Grade ONLY), the name of the school where they teach, the school address including City and State along with its telephone number. Internet entries will be deemed made by the authorized account holder of the email address submitted at the time of entry. The authorized account holder is the natural person who is assigned to the email address by an internet access provider, online service provider or other organization that is responsible for assigning an email address or the domain associated with the submitted email address. Multiple participants are not permitted to share the same email address. Entries submitted will not be acknowledged or returned. Use of any device to automate entry is prohibited. Proof of submission of an entry shall not be deemed proof of receipt by Stations. Stations’ computer is the official time keeping device for the Promotion. The Stations are not responsible for entries not received due to difficulty accessing the internet, service outage or delays, computer difficulties and other technological problems. There will be sixteen (16) “Entry Periods” during the Promotion Dates as follows.
Entry Periods begin at 5:00pm CT on the first date and end at 11:59pm CT on the second date. The dates for each Entry Period are as follows:
• From 8/26/2013 to 8/29/2013;
• From 8/26/2013 to 9/05/2013;
• From 8/26/2013 to 9/12/2013;
• From 8/26/2013 to 9/19/2013;
• From 8/26/2013 to 9/26/2013;
• From 8/26/2013 to 10/03/2013;
• From 8/26/2013 to 10/10/2013;
• From 8/26/2013 to 10/17/2013;
• From 8/26/2013 to 10/24/2013;
• From 8/26/2013 to 10/31/2013;
• From 8/26/2013 to 11/07/2013;
• From 8/26/2013 to 11/14/2013;
• From 8/26/2013 to 11/21/2013;
• From 8/26/2013 to 12/01/2013;
• From 8/26/2013 to 12/05/2013; and
• From 8/26/2013 to 12/12/2013.
c. All online entries must be received during the above deadline dates to be eligible for that Entry Period’s random drawing; all entry submissions will be automatically eligible for all future Entry Period drawings throughout the Promotion Dates. You do not need to resubmit your entry to be eligible for the next Entry Period’s drawing.
d. There are no limitations on how many nominations a teacher may receive during each weekly nomination period or how many times a teacher may be nominated. However, a teacher is only eligible to be named the Teacher of the Week one time.
2. ELIGIBILITY RESTRICTIONS
a. The Promotion is open to all persons who are 14 years of age or older who reside in the Stations’ Arbitron Metro area in Illinois, Indiana or Wisconsin. You may nominate only teachers who teach Kindergarten through 12th Grade in the States of Illinois, Indiana or Wisconsin, including all teachers in public and private schools. Employees of Northern Illinois University, the Stations, their licensee, each of their respective parent, subsidiary and affiliated entities, Stations’ advertising and promotional agencies, its participating sponsors, other radio stations in the Chicago metropolitan area, the members of their immediate families (spouse, parent, sibling or child) and those living in the same household of each (whether related or not), are ineligible to enter or win. If the teacher nominee is no longer teaching school or moves to a school not in one of the above-mentioned states or becomes ineligible then he/she will no longer be eligible for consideration for the Teacher of the Week. If a nominee is chosen as a Teacher of the Week and later quits teaching school or moves to a school not in one of the above-mentioned states during the Promotion Period, then he/she will no longer be eligible for consideration for the Grand Prize. This Promotion is subject to all applicable federal, state and local laws and regulations and is void where prohibited.
b. Persons are eligible to win a prize in a Promotion conducted by the Stations only once every thirty (30) days and only once every six (6) months if the prize is valued over $600. Only one (1) winner per household is permitted in any Station promotion.
c. The winner (the person who is nominated) must provide valid government-issued photo identification and provide their complete address, date of birth and phone number to claim a prize and sign and return any required Affidavit of Eligibility and/or Publicity/Liability Release in order to claim a prize.
d. All information submitted in nomination entry fields will be subject to verification. Entrants are required to provide truthful information and the Stations will reject and delete any entry that it discovers to be false or fraudulent. The Stations will disqualify any entry from individuals who do not meet the eligibility requirements. The Stations will also delete any entry knowingly received from persons under the age of 13 in compliance with the Children’s Online Privacy Protection Act.
a. A maximum of sixteen (16) Qualifying prizes will be awarded in this Promotion. Each Qualifying prize is one (1) $250 Visa Gift Card. The Approximate Retail Value (“ARV”) of the prize is $250.00. Each weekly Qualifying Prize winner (nominated person on entry) will be declared the “Teacher of the Week”.
b. One (1) Grand Prize will be awarded at the conclusion of the Promotion to one Teacher of the Week who will be selected at random from all previous selected Teachers of the Week. Grand Prize is one (1) two (2)-hour Chicago Bears player
Appearance (Player/Date TBD) at the Grand Prize winning teacher’s school. The player appearance will be coordinated between the Contest Entities and the appropriate school officials at a date and time to be mutually agreed upon. Grand Prize has no cash or other value.
c. The Teacher of the Week will be solely responsible for all taxes and all other fees and expenses not specified herein associated with the receipt and use of any Prize. Any Gift Card will be subject to the terms and conditions as set forth by the issuer of the Gift Card.
d. All prizes or prize certificates may be picked up at the office of the Stations at the address below. The winner will forfeit any prize or prize certificate not claimed within forty-five (45) days of winning. In the event that a prize or prize certificate is mailed to the winner, it will be with the prior written consent of the winner and therefore, winner assumes the risk of its loss. The Stations are not responsible for the safe arrival of a prize or prize certificate.
e. There is no substitution, transfer or cash equivalent for prizes, except that the Stations may, at their sole discretion, substitute prizes or cash of comparable value. The prizes are expressly limited to the item(s) listed above and unless otherwise expressly specified, do not include taxes, gratuities or any other expenses. Other restrictions may apply.
4. WINNER SELECTION AND NOTIFICATION
a. Decisions of the Station management with respect to the Promotion are final.
b. During the Promotion Period, a total of sixteen (16) potential weekly Qualifying Prize winners will be selected in random drawings. At the end of each Entry Period, the Station will select one (1) weekly Qualifying Prize winner through a random drawing from all entries received during the respective Entry Period and entries not selected in previous Entry Period(s). Each Weekly Prize will be awarded upon confirmation of eligibility. Entries not chosen during a weekly Prize winner drawing will be included in subsequent weekly Prize winner drawings. The Station will notify each weekly Prize winner by telephone within twenty-four (24) hours of the drawing. If no eligible nominations are received the first Entry Period, no prize will be awarded.
c. On Thursday, January 9, 2014, the Grand Prize winner will be selected by the Station through a random drawing from all weekly Qualifying Prize winners. The Grand Prize winner will be notified by telephone within twenty-four (24) hours of the Grand Prize winner drawing.
d. Odds of winning a weekly Qualifying Prize depend on the number of all eligible entries received up to the entry deadline on that week’s Entry Period. Odds of a weekly Qualifying Prize winner winning the Grand Prize are 1 in 16.
e. The winner does not need to listen or be present to win.
f. Each winner must execute and return any required affidavit of eligibility and/or liability/publicity release within five (5) days of notification attempt or prize will be forfeited and an alternate winner may be selected in a random drawing from the remaining entries received.
g. If a potential winner cannot be contacted, fails to sign and return the required affidavit of eligibility and/or liability/publicity release within the required time period, or if a prize or prize notification is returned as undeliverable, the potential winner forfeits the prize.
a. Payments of all federal, state and local taxes are solely the responsibility of the winner. The winner will be required to complete and submit an IRS Form W-9 with the winner’s full Social Security Number or the equivalent for receipt of any prize valued at $600 or more or for any prizes awarded by the Stations (or any other CBS Radio station in Station’s market) in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in forfeiture of the prize. Such winnings of $600 or more will be reported to the IRS.
b. By participating in the Promotion, the winner agrees to have the winner’s name, voice and likeness used in any advertising or broadcasting material relating to the Promotion without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
c. Prior to awarding any prize or prize certificate, the Stations, in their sole discretion, may require Promotion winner (and any travel companion(s) or guest(s)) to sign a liability release, agreeing to release and hold harmless the Stations, their licensees, each of their respective parent, subsidiary and affiliated entities, their respective officers, shareholders, directors, employees, agents and representatives and all of their successors and assigns from and against any and all claims or liability arising directly or indirectly from the prize and participation in the Promotion.
d. If for any reason this Promotion cannot be executed as planned, including, but not limited to, as a result of infection by computer virus, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Stations that corrupt or affect the security, administration, fairness, integrity or proper conduct of the Promotion, or if the Promotion is compromised or becomes technically corrupted in any way, electronically or otherwise, the Stations reserve the right to cancel, terminate or modify the Promotion. If the Promotion is terminated before the original end date, the Stations reserve the right to select the winners by random drawing from among all eligible non-suspect entries received as of the termination time/date or otherwise modify the procedure for selecting winners.
e. The Stations, in their sole discretion, reserves the right to disqualify any person tampering with the entry process or the operation of the Stations’ website. Failure to comply with the rules of the Promotion may result in a contestant’s disqualification and/or forfeiture of any prize or prizes. If the Stations make a good faith determination that an entrant has cheated or committed fraudulent activity in connection with a Promotion, the Stations reserve the right to disqualify that entrant from entering and/or winning future Promotions and to prosecute and seek damages to the fullest extent permitted by law.
f. The Stations reserve the right to make changes in the rules of the Promotion, including, without limitation, the substitution of a prize of equivalent value, which will become effective upon announcement. If due to circumstances beyond the control of the Stations, any competition or prize-related event or travel is delayed, rescheduled, postponed or cancelled, the Stations reserve the right, but not the obligation, to cancel or modify the Promotion and shall not be required to award a substitute prize.
g. The Stations are not responsible for typographical or other errors in the printing, the offering or the administration of the Promotion or in the announcement of a prize.
h. For a copy of these Official Rules, send a self-addressed stamped envelope for receipt by December 29, 2013 to “Teacher of the Week – Official Rules,” Attn: Promotions, WBBM, Two Prudential Plaza, Suite 1100, Chicago, Illinois, 60601. For the names of the prize winners send a self-addressed stamped envelope for receipt by February 28, 2014 to the above address marked “Teacher of the Week – Winner List.” The Official Rules and the Winner List (when completed) shall also be available during regular business hours at the main offices of the Station and may be posted online at http://www.cbschicago.com.
